Every aspect of the Emerge Programme’s medical delivery is overseen by a CQC-registered medical provider. Before any treatment begins, you will have been thoroughly assessed — medically and psychologically — and both Paul and Dr Devany will be satisfied that KAP is appropriate for your specific situation.

Medical Screening & Assessment

Before your first KAP session, you complete a comprehensive medical questionnaire and attend a face-to-face medical assessment with Dr Devany. This covers your full medical history, current medications, contraindications and any factors that might affect how you respond to ketamine.

Medical screening is not a formality. If ketamine-assisted psychotherapy is not medically appropriate for you, we will not proceed — and we will support you to find an alternative pathway that is.

CQC-Registered Medical Oversight

Medical assessment, prescribing, storage and administration of ketamine are provided through a separate CQC-registered medical provider — ensuring full regulatory compliance and the highest standards of clinical governance throughout your programme.

Dr Devany oversees all medical aspects of your treatment. Every ketamine dose is individually prescribed and prepared for your specific treatment plan. All medical monitoring during sessions is conducted to the standards required by the Care Quality Commission.

What Happens If Something Feels Wrong

During every KAP session, Paul is present throughout — available to provide support, adjust the approach or intervene if needed. Dr Devany’s medical oversight means that any physiological concerns during a session are managed immediately and appropriately.

After sessions, the integration process provides a structured space to process anything difficult that arises. Between sessions, you have direct access to Paul. If at any point during the programme either Paul or Dr Devany has concerns about your wellbeing, those concerns will be raised openly and your programme adjusted accordingly.

The Default Mode Network and Rumination

In depression, the default mode network becomes chronically overactive — the neurological basis of rumination and the loop of negative, self-focused thought that feels impossible to step out of. Ketamine temporarily quiets this overactivity, creating a pause in the narrative that no conventional antidepressant reliably achieves.

Glutamate, NMDA Receptors and Synaptic Renewal

Ketamine works on the glutamate system by temporarily blocking NMDA receptors. This triggers a rapid surge in glutamate release, stimulating AMPA receptors and strengthening synaptic connections in key mood-regulating circuits.

BDNF and Neural Regeneration

Chronic depression is associated with reduced BDNF expression, leading to loss of dendritic density and reduced hippocampal volume over time. Ketamine rapidly increases BDNF expression, promoting the growth of new dendrites and synaptic connections.

The Psychotherapy Window

These neurological changes create a window of heightened neuroplasticity and psychological flexibility that typically lasts hours to days. During this time, the brain is more open to forming new patterns — this is when the therapy in KAP does its most important work.
