Why Conventional Treatment Isn’t Always Enough
Most people who come to The Emerge Clinic have already tried a great deal. They may have taken several antidepressants, sometimes for years. They have often been through therapy, read extensively and worked harder than the people around them realise to understand what is happening.
However something remains stuck. That is not because they have failed or have not tried hard enough. Depression, anxiety and trauma are complex, and no single treatment or biological mechanism explains every person’s experience. Conventional medication and psychotherapy help many people, but others continue to struggle despite receiving appropriate care.
Conventional antidepressants target the serotonin system. For roughly a third of people with depression, that’s enough. For the rest — those with treatment-resistant conditions, deeply embedded trauma, or neurological patterns that serotonin-targeting treatments simply can’t reach — something different is needed.
The Emerge Programme was designed for that group. The people who haven’t given up, but who need a different kind of intervention — one that works at a neurological level other treatments can’t access, supported by the psychological expertise to make that change last.

What Is the Emerge Programme?
The Emerge Programme is our structured month-long ketamine-assisted psychotherapy programme, designed for people whose depression, anxiety or trauma has not responded sufficiently to conventional treatment.
It combines four medically supervised KAP sessions with dedicated preparation and integration work — ensuring that every moment of clarity from the medicine is translated into real, lasting change in how you think, feel and function.
What makes the Emerge Programme different from standalone ketamine infusion is the psychotherapy that surrounds every session. The medicine creates a period of greater psychological flexibility — a state in which new patterns of thinking and feeling can take hold more readily. The therapeutic work before, during and after each session is what you do with that window. Without preparation and integration, much of the therapeutic opportunity may be lost.
This is not a quick fix. It is a carefully designed process that asks something of you — preparation, intention, reflection and integration. For people who are ready to do that work, it represents one of the most significant therapeutic opportunities currently available in the UK.
The Four Stages
Every stage has a purpose. Nothing happens by accident.
INTAKE & ASSESSMENT
Before anything else, we need to understand you. The intake process includes a detailed medical and psychological questionnaire followed by assessments with Paul and Dr Devany. We review your treatment history, current difficulties and what you hope to achieve, while also making sure ketamine-assisted psychotherapy is medically and psychologically appropriate.
Duration: Typically one to two weeks before the programme begins.
PREPARATION
Preparation is not a formality. It is one of the most clinically important parts of the programme. Preparation helps establish trust, clarify what you hope to explore and create a stronger therapeutic foundation for the sessions that follow.
In the preparation phase, you and Paul work together to identify what you’re carrying, what you’re hoping to release or understand, and how to approach the sessions with openness rather than anxiety. You’ll explore your history, set intentions for each session, and develop the trust that allows you to go deeper during the medicine work than you could with a stranger.
This phase also includes practical preparation — understanding what to expect during a session, how to work with the experience rather than against it, and how to approach the integration work that follows.
Duration: Preparation sessions take place in the weeks before your first KAP session.
KAP SESSIONS
Each ketamine-assisted psychotherapy session follows the same carefully structured format: a clinical check-in, review of your individually prescribed dose, a brief intention-setting conversation and administration of the medicine by intramuscular injection.
The treatment experience lasts approximately two and a half hours. You are supported throughout, given time to recover afterwards and encouraged to note anything important while it is still fresh. A personalised guided meditation is then created to support the neuroplastic window in the days following the session.
Duration: Four sessions across approximately four weeks.
INTEGRATION
Integration is where the therapeutic work happens. The KAP sessions create the material — insights, emotions, memories, shifts in perspective — and the integration sessions are where you make sense of them, work through them, and begin applying them to how you actually live.
Integration is where the experience is explored, understood and connected to your life. Each treatment session is followed by a dedicated integration appointment with Paul, usually within a few days.
These are structured therapy sessions in which the content of the experience is considered alongside your history, present difficulties and established patterns. Integration helps translate insight into practical psychological work and allows the therapeutic approach to be adjusted as the programme develops.
Duration: Four integration sessions — one following each KAP session, typically within a few days of each.
The complete programme — including assessment, preparation, four KAP sessions, four integration sessions and ongoing support — is included within one fee. There are no additional treatment charges.

A comprehensive Emerge journal workbook, plus carefully selected neuroscience-informed exercises, to help you process each session, track what is changing and continue the therapeutic work between appointments.
Direct contact with Paul between sessions throughout the programme — for questions, concerns or anything that arises as you move through the work.
Some people — particularly those with longstanding or treatment-resistant conditions — benefit from extending beyond the standard programme. Additional sessions are available at £595 each, including a dedicated integration session. Four additional sessions booked together receive a 10% discount.
Sessions within six months of your last require a medical questionnaire. Sessions beyond six months require a full medical consultation.

The only way to know for certain is through our assessment process. We review your full medical and mental health history, background and your current situation before making any recommendation. If we don’t think KAP is right for you, we will tell you clearly — and where possible suggest what might be. The assessment is the starting point, not a formality.
No. You can self-refer directly to The Emerge Clinic. However, we do ask that you inform your GP that you are undergoing KAP treatment, and we are happy to liaise with your GP or existing mental health team if you would find that helpful.
The programme runs over approximately one month. The exact timeline depends on your scheduling and how you respond between sessions — some people move through more quickly, others benefit from slightly more space between sessions. We work to a pace that serves you, not a fixed calendar.
The Emerge Programme is designed for people who have tried conventional treatment without sufficient relief — but that doesn’t mean you need an extensive therapeutic history to be considered. What matters is that you are ready to engage seriously with the therapeutic process. We assess readiness as part of the intake process.
No. You must not drive on the day of a KAP session. You will need to arrange transport to and from the clinic. We ask that you have someone available to collect you after each session — both for safety and because having support around you in the hours following a session can be beneficial.
Difficult experiences during KAP sessions are not uncommon — and they are not necessarily bad therapeutically. Paul is present throughout every session and is experienced in supporting people through challenging material. If something feels overwhelming, you can communicate with Paul at any time. The session environment is designed to be safe and contained, and difficult experiences are always worked with therapeutically rather than suppressed.
This depends entirely on your specific medications and medical history. Some medications interact with ketamine and may need to be adjusted or paused under medical guidance before treatment begins. This is assessed individually by Dr Devany during the medical assessment. Never adjust your medication without medical advice.
No. The Emerge Programme requires in-person attendance at Progress House, Gorleston-on-Sea, Norfolk. The KAP sessions must be conducted in person for both medical and therapeutic reasons. Some preparatory conversations may be conducted online at Paul’s discretion, but the programme itself is in-person only.
The programme concludes with a progress review — an honest assessment of outcomes and recommendations for next steps. For some people, the four sessions are sufficient for meaningful and lasting change. Others benefit from additional sessions at £595 each. Paul will make an honest recommendation based on your individual progress — never based on commercial considerations.
